CONDITIONS OF ENTERTAINMENT AGREEMENT

1. A KB Pro DJ will provide music and lighting during the times outlined in this agreement depending on the package ordered

2. A KB Pro DJ will make all announcements as requested by client.

3. Client will provide a cover for a 2' x 6' table that matches decor.

4. Unlimited time...when folks start cleaning up and the dancing quits we consider the party to be about over...

By paying with credit card you are accepting the agreement as filled out. This creates a binding contract between us. After completing the credit card payment (secure through PayPal), click Print button below to print out this agreement, then click on Submit button to forward a copy to KB Pro DJs.

CHECK:

Make check out to KB Pro DJs. Click Print button below to print out the agreement and keep for your records. Send the check to KB Pro DJs, 4004 Grouse Dr., Evans, CO 80620. Click submit to forward a copy to KB Pro DJs. This holds the date for 10 days while your check gets to KB Pro DJs. Your sending a check with time and date filled in the memo portion, and KB Pro DJs signing and cashing it makes a binding contract between us as per the agreement you filled out. KB Pro DJs may also assign another KB Pro DJ for events in case of illness or accident.

This Entertainment Agreement supercedes all.
